Default JPP Fish Attractors

Its my understanding that i need to get the free permit from the Corps to put fish attractors on Priest so i went to thier website, clicked on the link that is suppose to take me to the form to fill out and its a dead link. So i called thier office that is right behind the parking lot at the dam and the lady told me i can swing on in there and pick one up. That is all fine and good but thier business hours dont jive with the average working mans hours. LOL.
Has anybody tried this before that may have that PDF thats not filled out they can send to me or a different link than i tried or possibly any other idea's? I asked the lady if they would be willing to email me or even snail mail me one and she just repeated the "come on into the office and we will give ya one" HA! lol.
With the lake dropping and Mr. TK working on my motor, its perfect timing to drag some brush and blocks out there and set my crappie traps for spring! LOL
